This is a clip from the good old days at Mount Panorama, Bathurst NSW Australia and the Hardie Ferrodo 1,000. This is Dick Johnson flying around in his XE Falcon. It ran a 351 Cleveland V8, 4 speed Top Loader and 9" diff. He was on a flyer until 2 turns before Conrod Straight, when things went wrong. http://www.youtube.com/watch?v=WpNqdpVebMY

Dick is one of our local touring car heroes over here. Given his age, neither "term" were probably in common use when he was born.
No he was not killed. In fact he only suffered minor injuries. Cut's bruises etc. The car was a write off though. I remember the incident from watching it as a kid. Back in the early 80's he had two major wrecks on the mountain and this was one of them. If memory serves me correctly, another racer that year actually gave him a car to race that weekend. So they did a mad all night trash and fronted the start line the next day, with the rebuilt drive line from the wrecked car installed and a brand new paint job. The paint was still drying as the race started.

The problem was that someone installed the steering wheel on the wrong side.
